To all schools and colleges:
In order to promote the construction of the "One Form" project, we solicit the third group schools and colleges of the university to use "One Form" platform in 2020. The relevant information is hereby notified as follows:
I. Introduction to "One Form" project
The "13th Five-year Plan" Informatization Development Planning of Huazhong University of Science and Technology proposed the construction of "Ten Ones" projects, including this "One Form" platform. The "One Form" project mainly solves outstanding problems strongly concerned by faculty and students, such as scattered, inaccurate and inconsistent university data and forms to be filled repeatedly.
This project mainly includes the basic database, data sharing and exchange platform and "One Form" platform of the university. Among which, the "One Form" platform realizes the functions of collecting personal data of faculty and staff automatically, maintaining them in a concentrated way and displaying them visually, personalizing the appraisal plan of schools and colleges, automatizing the calculation of performance distribution and creating reports automatically, etc. At present, the platform has integrated 70 data subsets in five categories of teaching, scientific research, theses, human resources and public welfare. With the platform, faculty and staff can maintain their personal data, fill in their performance and check the visual analysis result of their personal data. Schools and colleges can manage faculty' data and performance by category and check the visual analysis result of various data. The system can effectively reduce the burden of faculty and staff in filling in data, realize information-based management of personal performance data and annual assessment of faculty and staff, and improve the information-based management level of schools and colleges.
Since its put into use at the end of 2018, the platform has been used on following pilot schools for two periods and achieved good results: School of Mechanical Science & Engineering, College of Life Science & Technology, School of Environmental Science & Engineering, School of Computer Science & Technology, School of Basic Medicine, Tongji Medical College, School of Sociology, School of Mathematics and Statistics, School of Physics, School of Artificial Intelligence and Automation, School of Chemistry and Chemical Engineering and Network and Computing Center. Starting from 2020, we plan to generalize the "One Form" platform progressively and orderly throughout the university. In addition to the above eleven schools that have participated in the pilot, another six schools across the university will be solicited to use the platform.
II. Application procedures
The applicant designates a contact person to fill in the "Application Form for Using 'One Form' Platform" on the online service hall (website: https://ehall.hust.edu.cn/infoplus/form/YZBGCSDXYSQ/start ).

IV. Selection process and follow-up affairs
Network and Informatization Office will select schools and colleges by taking into account their order of submitting application forms, their disciplines and other factors.
Each applicant shall establish a working team with the college leader as the team leader and with leaders of teaching, scientific research, graduate, human resources and other departments as team members to coordinate the application of the platform. They also need to submit the basic information of course teams, scientific research teams, internal institutions, party branches and other relevant personnel, as well as documents related to their year-end assessment (assessment plans, documents, etc.).
